PLEASE PUT SIGNATURE IN THE MIDDLE OF BOX ON UPPER RIGHT- HAND CORNER OF FIRST PAGE OF APPLICATION
1. Applications MUST be handwritten, using a pen and in block capital letters.
2. Applicants MUST complete Sections 1, 3, 4, 6, 8, 10 and 11.
- Applicants who are married/divorced must also complete Section 2
(Please submit divorced or marriage certificate for name change).
- Applicants born outside of Grenada must also fill Section 5.
- Applicants Replacing Passports (Lost or Stolen) must complete Section 7.
3. Section 12: Recommender MUST be completed by a Grenadian (not a relative) who knows the applicant for at least 3 years.
4. Children under the age of 16 years mush have written consent of their parent or legal guardian when applying for passport. (section 9 must be completed)
5. All applications MUST produce original Birth Certificates or Certificates of Naturalization or Registration and surrender previous passports with application.
- In case of lost passport applications, a Police Report and Affidavit must be submitted. (the affidavit form can be obtained from the Consulate or can be down loaded from our web site www.grenadaconsulate.org)
- Applicants for lost passport must obtain a letter from the Consulate in order to obtain a police report/incident slip.
6. Two recent passport size pictures must be submitted with application.
Recommender MUST endorse the reverse side of one photo with the words: "I certify that this is a true likeness of the applicant Mr. (Mrs. or Miss)......"and add his signature. |
